Most athletes can begin ACL physical therapy within the first week after surgery, starting with gentle range of motion and swelling management. Our most common ACL package is 40 visits over 9 months, with monthly or quarterly monitoring over the first 12 months after return to sport. We use objective criteria to individualize everyone's rehab program and plan, not time alone. Most athletes will return to limited participation in practice around 6-7 months and initially return to competition around 9 months with some restrictions. However, every athlete is different. We use objective testing at each phase to ensure you're truly ready to progress, never rushing the process or holding you back unnecessarily. Athletes typically continue to see improvements for up to 24 months after injury as they return to their full prior level of performance.
Return-to-sport decisions are never based on time alone. We use comprehensive testing including strength measurements, force plate testing, 3D movement analysis, movement analysis, psychological readiness assessments, and sport-specific drills. You must demonstrate at least 90% strength symmetry between legs, proper landing mechanics, and confidence in cutting and pivoting movements. Our goal is ensuring you return not just cleared to play, but truly prepared to perform.
ACL re-injury prevention starts during rehabilitation, not after. We focus heavily on neuromuscular control, proper landing mechanics, and identifying movement patterns that put you at risk. You'll learn how to recognize fatigue-related form breakdown and develop the strength and awareness to maintain safe movement patterns even when tired. Many of our ACL athletes continue with our performance programs to maintain these protective adaptations long-term.
With proper rehabilitation, most athletes not only return to their previous level but often perform better than before their injury. ACL physical therapy addresses weaknesses and movement dysfunctions that may have contributed to the original injury. You'll develop better body awareness, improved strength balance, and more efficient movement patterns – often resulting in enhanced performance and reduced injury risk across all activities.
